The screen shot/statement must show - 1. it is within the last 3 months, 2. the Bank Name, 3. Account Name, 4. Account Address, 5. Sort Code and 6. Account Number. It does not need to show any transaction details. You will be asked to upload the screen shot/statement when completing this form, so you should have it ready.
